Students
Filling out this form will help us collect data and if you wish for a personal response and/or community or individual harm restoration, a menu of options is available. Only incidents outside of academic engagement (academic engagement includes any pedagogical, research and/or educational activities) should be reported. Those that fall under academic engagement should be reported to the dean of school (or their designee) connected to the incident.
- Undergraduate students in distress or wanting an immediate response, contact your local neighborhood student support staff (RA/House Staff).
- Graduate or coterminal students in distress or wanting an immediate response, contact a Graduate Life Office (GLO) Dean
If you want to connect with a confidential office, you can contact the Office of the Ombuds, Confidential Support Team, Counseling and Psychological Services (CAPS), or the Office of Religious and Spiritual Life.
